Team Care Approach
• For optimal success with the Bredesen Protocol, Dr. Semby recommends working with her ReCODE 2.0 certified care team which includes Michael Sanders, Health Coach, Dr. Erela Rappaport, DMD, dentist.
Commitment
• This is a program of mutual commitment. Reversing early stages of cognitive decline requires hard work and perseverance. We recommend patients sign up with us for a year at a time as this way we can work with you consistently to ensure you get the results you desire.
• In the first year, we work with you very closely to teach you how to follow the program, how to monitor changes in your health and getting labs done at a certain frequency. In subsequent years, you need us lesser and the time between visits is increased.
Active ReCODE Account
• Patients must maintain an active ReCODE account with Apollo Health. The cost of the ReCODE account is not included as part of the cost of care at the Center for Cognitive Resilience.

The Journey Begins with Center for Cognitive Resilience
- Your journey begins with a Meet and Greet (45 minutes, $150) consultation with Dr. Manna Semby (she is MOCA certified) when she administers a cognitive assessment to establish a baseline.
- The focus is for you to learn about Dr. Semby and for her to learn about you. The most important thing to assess is your willingness to adapt to a new lifestyle program. We also help you decide which program is right for you: ReCODE or PreCODE.
- Our offerings for ReCODE and PreCODE start with an intensive 12 months of work together. After the first year, we have ongoing care options available so we can continue to support you and keep you motivated on your ReCODE or PreCODE journey.
Engage
Sign up with Apollo Health, do baseline blood labs through them, generate your report
- The next thing is to sign up with Apollo Health for the ReCode or PreCODE Programs at the Apollo website https://www.apollohealthco.com/. Membership is $75/month (for ReCODE) or $40/month (for PreCODE), respectively, paid annually, directly to Apollo Health.
- Purchase and complete registration for the baseline lab panel through Apollo Health (including entering your doctor’s name and contact details). The current cost for the ReCODE Baseline Lab Panel is $855. The current cost for the PreCODE Baseline Lab Panel is $334.
- Complete the Medical History Questionnaire on the Apollo portal and Cognitive Assessments (specific to your program type).
- Once your results are back, you can automatically generate your ReCODE or PreCODE Report.
Select Dr. Manna Semby as your practitioner on the Apollo Health Portal, get a detailed plan based on Apollo and Dr. Semby’s recommendations
- “SELECT” Dr. Manna Semby, ND, IFMCP, as your practitioner and complete the Practitioner Affiliation Agreement on the Apollo portal. This connection enables her to access your reports, assessment and lab result.
- Meet with Dr. Semby to review your personalized ReCODE or PreCODE Report results, discuss the findings in detail, make recommendations for diet, lifestyle, supplements, and prescriptions, if needed.
Get Custom Functional Labs with Dr. Semby
- Dr. Semby may recommend additional functional labs* to understand contributors to cognitive decline and your burden of toxins, gut issues, latent infections, and inflammation.
- Development of a personalized treatment plan based upon your ReCODE or PreCODE Report, which will include addressing root causes of cognitive decline, repeat labs and cognitive assessments at six months, health monitoring* for oxygen saturation, glucose and ketones, and guidance with dietary and lifestyle strategies.
Start Working with our Health Coach
- Once the test samples* (urine, stool, blood) are submitted, Michael Sanders, our health coach, starts working with you on managing stress, eating right, tracking glucose* and ketones*, movement opportunities, optimizing sleep hygiene*, and utilizing brain exercises.
- Through the entire program, you work with Michael to implement strategies laid out in your Plan. Any time you feel you are beginning to falter in your ability to follow the plan, Michael helps you to get back on track.
Oral and Sleep Health Screenings
- Next, we will send you to Dr. Erela Rappaport, DMD, our ReCODE 2.0 Certified Dentist, for an oral and sleep health consult. She will perform a dental screening exam** and provide instructions for a home sleep study** to screen for sleep apnea.
- Based on the findings of the oral and sleep screenings, Dr. Rappaport will recommend further dental work** and options for treating sleep apnea.
- You will meet Dr. Semby about once a month at the beginning of the program. Once you have gained a good familiarity with the program, the visits can become less frequent.
- About three months after the functional labs, you meet with Dr. Semby to provide an update on how you are doing and compare your symptoms to baseline.
Evaluate
Lab Reviews and Cognitive Assessments
- Continuous evaluation of your symptoms and cognition helps us understand if the treatment is achieving its goal. Your Plan is edited and updated as necessary.
- At the first meeting to discuss results of labs, a long-term strategic Plan is created for you. Then sections of it are prioritized for a period of 2-3 months each while you work on specific aspects such as the gut, or detox, or reducing inflammation.
- Review of cognitive assessments results (SLUMS or MoCA, CNS Vital Signs or Cq Assessment, and the AQ-21) is done regularly over time to track your performance and progress.
- Continue to do Brain HQ and monthly cognitive assessments through ReCODE. You may also join various support groups through the website.
Retest Blood and Functional labs
- New sets of labs are drawn (around 6 months after your baseline tests) to understand how your body is responding to the treatment.
- Once the new labs results come back, we meet again to review and adjust your personalized treatment plan.
Continue Working with Care Team
- You continue to work with the care team to ensure you have the help and support you need.
- Communication between visits to provide support as things come up (Simple and direct questions can be answered via this practitioner’s portal. Questions that are more involved should be answered in a visit.)
